NOTICE OF COMPANY
DISSOLUTION
Notice is hereby given that HJM, LLC, an Indiana Company, has been dissolved. Any person or entity with a claim against said Company must provide the following:
1. Name, address and telephone number of claimant;
2. A statement regarding the basis of the claim; and
3. the amount of the claim.
The notices of any claims should be mailed to Company’s counsel as follows:
